Q. Will my name or personal information be disclosed to any person or company outside of DMD?
A. No. Your name, address, and all other personal information will remain strictly confidential within DMD. (See Confidentiality Statement)
Q. How often will I be asked to participate in surveys after I become a panel member?
A. Frequency will vary according to the demand for contacting patients using specific prescription medications. You may not ever be contacted, or you may be asked to participate several times per year.
Q. How much will I be paid to participate?
A. Assuming full participation, you will be compensated $5 to $25 according to the type of survey. Payment amount will be provided to you in advance. A check for the specified amount will be mailed after you have participated.
Q. What if I discontinue using the prescription medications indicated on the enrollment form?
A. DMD will periodically send an update form to you to keep our records current.
Q. Can I drop my name off the Patient Panel at any time?
A. Yes. Simply indicate by E-Mail (this Website) your choice to terminate, and DMD will remove your name.
